What are bunions?
Bunions are bony protrusions located at the base of the big toe that develop when the toe is slanted inward or overlaps the next toe. They can be very painful. Bunions form when the movement of the big toe influences the angle of the bones in the foot. The changes gradually develop into the characteristic bump, which over time becomes more and more noticeable.

What are the signs and symptoms of bunions?
In addition to the typical bump, signs of bunions can include red, calloused skin along the foot at the base of the big toe. With bunions, you may also develop calluses on the big toe, sores between the toes, ingrown toenail, and restricted motion of the toe. Some bunions are small and painless and some are large and extremely painful. Pressure from shoes worsens the problem.

What are the causes of bunions?
The main cause of bunions is excessive pressurebeing placed on the front of the foot, and is usually the result of wearing high-heeled shoes with pointed toes. A study by the American Orthopaedic Foot and Ankle Society found that 88 percent of women in the United States wear shoes that are too small and that 55 percent of them have bunions. Overall, bunions are nine times more common in women than men. In some cases, bunions are hereditary; they also may be caused by arthritis or polio.

What is the conventional treatment of bunions?
Making sure that shoes don't press against the bunion worsening the pain is the first line of treatment. Protecting the bunion with felt or foam pads or devices to separate the first and second toes at night may be recommended as may cutting a hole in a pair of old, comfortable shoes to take the pressure off the bony protrusion. Nonsteroidal anti-inflammatory drugs may be recommended to help relieve toe pain. In rare cases, physicians may administer injections of corticosteroids to treat the inflamed bursa (fluid-filled sac located in a joint) sometimes seen with bunions. Custom orthotic devices are another option that may be beneficial in some cases.

If bunions are causing severe foot pain or inflammation and swelling that limits daily activities and doesn't improve with rest, medication and comfortable shoes,surgery may be required. More than 100 surgical options are available for painful bunions. Some realign the foot's anatomy by cutting notches from the metatarsal bone or the bone of the big toe. The bones can then grow back without the slant that promotes bunion growth. The operation is usually done on an outpatient basis, but afterward, you probably will have to stay off your feet for a few weeks. Recovery takes about six weeks. Surgery is not recommended for a bunion that doesn't cause pain.

What therapies does Dr. Weil recommend for the treatment of bunions?
Wear comfortable shoes that don't squeeze your toes together. Plantar fasciitis is the most commonly occurring heel pain seen in runners, obese people and pregnant women. The thick band of tissues in the bottom of your feet get inflamed, causing pain. The heel pain is usually felt on the inside of the heel. The pain is also felt along the arch of the feet and along the border of the heel. You feel a stabbing pain, especially, in the morning as the plantar fascia tightens up. The pain reduces as the tissues stretch, but it may worsen if you stand, walk or run. This condition is seen in athletes, dancers and jumpers.

Heel pain is usually felt in two main sections of the heel, under the heel and in the back of the heel. In most cases, when people suffer from pain in the back of the heel it is related to overuse or inflammation of the Achilles tendon. When people suffer from plantar fasciitis pain, it can be caused by a person stepping on a large stone or rock which bruises the heel, or it can be caused by inflammation of the plantar fascia. A heel that is hurt by a sharp object will heal on its own over time.

Is there a difference between Plantar Fasciitis and heel spurs? While there is a difference between the two conditions, they are related. Plantar fasciitis is a condition where the thick tissue on the bottom or your foot becomes irritated and swollen. Plantar Fasciitis means inflammation of your plantar fascia. The plantar fascia is tissue that holds up the bones on the bottom of your foot. When you have this condition you usually feel pain in the bottom or your heel. This may be due to arch problems. When your plantar fascia pulls away from the heel, calcium deposits can form there.

Pain can sometimes cause a lot more than just discomfort. It can often significantly impact daily activities since any weight placed on the affected area can deliver serious pain which can prevent you from doing daily activities and exercise. Plantar fasciitis causes an aching pain that can be localized in the heel, but also radiate throughout the foot. In most cases, pain is most noticeable and serious in the morning when getting out of bed, or after standing up after prolonged sitting. This is because pain in the inflamed area subsides after the plantar fascia relaxes.

An easy home exercise for plantar fasciitis involves the use of a tennis ball or any small ball that is comfortable to use on the bottom of the foot. The exercise is performed by placing the bottom of the foot on top of the ball and gently rolling the ball back and forth. This is thought to massage the muscles and stretch the muscles along the sole of the foot to relieve tension. This can be performed while seated or standing while holding on the a wall or chair. The exercise can be performed for 30 seconds to a minute at a time followed by a period of rest. plantar fasciitis shoes

The plantar fascia is a thick fibrous band of connective tissue originating on the bottom surface of the calcaneus (heel bone) and extending along the sole of the foot towards the toes It has been reported that plantar fasciitis occurs in two million Americans a year and in 10% of the U.S. population over a lifetime. Another symptom is that the sufferer has difficulty bending the foot so that the toes are brought toward the shin (decreased dorsiflexion of the ankle ). The two muscles that we call the calves (Gastrocnemius and Soleus) attach to the heel via the Achilles Tendon. The Achilles Tendon wraps over the heel bone where it then becomes the Plantar Fascia. The Plantar fascia stretches across the bottom of the foot to the base of your toes. While we may think of these muscles and tendons as separate tissue structures , you can see by the picture that these structures are not separate They are one continuous fascial tissue structure. So you can imagine that tension in one will affect each of the others.

Other effective techniques include use of a slant board ( Figure 3 ) or placing a two-inch ├Ś four-inch piece of wood ( Figure 4 ) in areas where the patient stands for a prolonged time (e.g., workplaces, kitchen or stoves) to use in stretching the calf. Dynamic stretches such as rolling the foot arch over a 15-oz size can or a tennis ball are also useful ( Figure 5 ) Cross-friction massage above the plantar fascia ( Figure 6 ) and towel stretching ( Figure 7 ) may be done before getting out of bed and serve to stretch the plantar fascia.

Why is this exercise valuable? The muscles of the feet require good strength to control the forces associated with landing on the ground during the running stride. This toe-walking exercise helps to develop the eccentric (support) strength and mobility in the muscles of the foot and calf, as well as the plantar fascia and Achilles tendon (eccentric strength means hardiness as these structures are being stretched out). The exercise also works the foot and ankle through a broad range of motion, especially for the foot which is bearing weight on the ball and toes while the ankle is extended (is in plantar flexion).

If you happen to strain or even injure your Plantar Fascia, you will know due to the fact Plantar Fasciitis is an extremely painful affliction of the feet. The Plantar Fascia is the muscle that runs from the heel to the ball of the foot. When this muscle is over-stressed or possibly been injured, when you first try to get out of bed each morning you will experience a new volume of pain.
